Exactly as shown - perfect for my tragus. Difficult to hold the tiny gem and too small for my grabber tool - I ended up putting gem screw side up into a small piece of new Blutac which was then so easy to hold and screw into post.
Advantages: Being able to change gem on the front without changing the post.
Disadvantages: Tiny gem difficult to hold but I assume piercer would fit for you or try my Blutac method above.
